Declare/Change Undergraduate OR Graduate Major/Minor/Other Credentials [you will be prompted to sign in to portal to access form]  Complete this form if you wish to change your major.  

Course Substitution Form: Complete this form if you are planning to take a course outside of the university.  You must submit this form with a copy of a syllabus of the class that you are requesting to take from an accredited AASCB university to mba@ship.edu.  This needs to be done before any transfer credit will be accepted from another university.  

Employer Reimbursement Form:  Have your employer complete this form and forward to Student Accounts (Studentaccts@ship.edu) to delay payments that will be made by your employer. 

Graduate Appeals Form and Graduate Appeal Instructions:  Complete this form and return to mba@ship.edu  

Incomplete Grade Request Form:  Please complete this form if you are requesting an incomplete grade and forward to mba@ship.edu.  

Request for Registering over 12 Graduate Credits in a semester:  Complete this form if you plan to take more that the permitted 12 semester hours in the spring or fall semesters. 

Override Request:    

This form is to be completed for the following reasons:

          a.   You received an error message while you were trying to register for a
                course or waitlist.

          b.   You are a CONDITIONALLY ADMITTED student that needs to register
                for a course.

          c.   You are an MBA Professional Student that wants to take an MBA
                Online course. 

          d.   You are an MBA Online Student that wants to take an MBA
                Professional course.

5 Year MBA Program for undergraduate Accounting Students (Application) and 5 Year Plan Worksheet (Instructions):  Please complete and submit all material to the Chair of the Accounting Department (GRH 328) for the initial signature and then to the Director, MBA Program (GRH 320).
