Available to

Qualifying families and individuals with income below $53,000

Items to Bring

  • Valid picture ID. (i.e. driver’s license)
  • Original Social Security Card or Individual Taxpayer Identification Number (ITIN) for you, your spouse, your children & other dependents included in your tax return
  • Copies of all forms W-2, 1098, 1099 and 1095-A (year-end tax form you have received in the mail)
  • Bank Routing and Account Numbers for Direct Deposit or a canceled check
  • Interest and dividend statements from bank
  • Total paid for day care AND day care provider’s tax ID #
  • Current Year Local Tax Return
  • Copy of 2013 Tax Return
  • *Filing a joint return? Both spouses must be present to sign the required form*
