Qualifying families and individuals with income below $51,000
Items to Bring
- Proof of Identification (i.e. driver’s license)
- Social Security Card and Birth Date for yourself, spouse, & dependents
- Wages & Earning Statements(W-2, W-2G, 1099-R from all employers)
- Bank Routing and Account Numbers for Direct Deposit
- Interest and dividend statements from bank
- Total paid for day care AND day care provider’s tax ID #
- Current Year Local Tax Return
- Copy of 2011 Tax Return
- *Filing a joint return? Both spouses must be present to sign the required form*
