Required Forms

New and returning International Students must submit personal and health insurance information to the Office of International Programs each semester by the end of the first week of classes. The required information can be submitted online by accessing and completing the forms below.

You must also send a photocopy of the health insurance card your company has issued to you, both front and back, to the Office of International Programs, no later than the Friday of the first week of classes. You may email the copy of your insurance card to April Ashway-Railing (AIAshwayrailing@ship.edu) or bring your insurance card in person to:

Shippensburg University
International Programs Office
Center for Global Education
Ceddia Union Building. Room 221
Shippensburg, PA 17257

Failure to submit the required information and provide proof of valid health insurance will result in a hold on your account which will prevent scheduling of classes and/or for graduation.
