Facilities maintenance requests submitted through this form are sent via email to your RD. The RD or RAs will take the information and enter it into the Facilities work request system. Work will be coordinated in your room to make the repairs in a timely manner. You can include your phone number in the Student Contact Phone Number field. This will be added to the work request by your RD or RA. The Facilities Department may contact you directly if there is a question or issue”.
