Online Education Complaint Form

Students who have a grievance, complaint, or concern related to a particular class should FIRST contact their faculty member to resolve the matter, in accordance with the procedures and policies outlined in Shippensburg University’s undergraduate and graduate catalogs.

Students who have grievance, complaint, or concern unrelated to a particular online class should complete the following form.  (Note that grievances, complaints, or concerns submitted via this form with regard to a particular class will be forwarded to the faculty member.)  You will receive an acknowledgment of the receipt of your submission within two business days of your submission of this form.  For more information on the use of this form, refer to the Online Education Complaint Process website or contact Shippensburg University’s Office of Professional, Continuing, and Distance Education at pcde@ship.edu or 717-477-1502.
