OPTICAL MARK READING SERVICE

STUDENTS SHOULD NOT TO USE THEIR SOCIAL SECURITY NUMBER
IN THE IDENTIFICATION AREA OF THE OPSCAN FORM.

Optical Scanning Equipment Information

  • The NCS Pearson OpScan 10 Model 40 Scanner is located in the Computing Technologies Center and consists of a scanning unit, microcomputer, and printer which interprets pencil marked forms at a rate of 4,000 sheets per hour and prints the results on the form.  The scanned information is also read to a disk file.  This disk file can be used as input to analysis and grading programs on the mainframe computer.

Standard Answer Sheet Forms

  • For test scoring purposes there is a supply of "Standard Answer Sheets" designed for use by our installation.  The sheets can be secured at the Central Warehouse.  These forms are not available at the CTC.

 

  • For tasks of a special nature where the standard answer sheets are not suitable, consultation at the CTC on form design is necessary; otherwise, the forms may not be readable by the scanner.

Marking Responses on Answer Sheets

Please inform your Students NOT to use their social security number OR ANY PART OF IT in the Identification Area of the Opscan form.

  • All forms are to be marked by a #2 pencil only.  All other marking media are not suitable for scanning.

 

  • Care should be taken to mark all forms with a uniform dark density of the #2 pencil.  The response area must be completely filled.  Be careful not to put marks between rows.

 

  • All erasures must be cleanly done.  A double response will be treated as a wrong answer, unless multiple responses are allowed.

 

  • Student name area should be filled in; the identification area can be used, but do not use the student's social security number.

 

 

  • Student forms must not have any information entered in the year fields, or scoring of exams will be affected. 

 

 

  • The Special Codes sections should be left blank on student forms, except when taking advantage of the 'Previous # of points to be added' option, in which case the N, O, and P fields may be filled in with the appropriate point values for each student.

 

  • Immediately following a testing session the instructor must make a cursory examination of the recorded information to be sure
    all marks are recorded properly, and all forms in the stack are in the same orientation.

 

  • Do not staple, tape or fold any sheets together; doing so will jam the scanner.  Extra care in handling forms will insure a good scan of the responses.

Submission Standards

  • For all scanning of forms the user is to take the forms to the box found in the hallway to the right of the MCT 054 computer lab entrance, outside the CTC doorway. Forms can be dropped off in the box marked Opscan Drop Off at any time and results should be ready within 1 to 2 business days.  Opscan test scoring will occur between the hours of 8:30 a.m. - 9:00 p.m.  Monday through Friday.   Please put your last name on the outside of the envelope.

 

  • A "Computing Technologies Center Opscan Request Form" is to be completed by all users.  This is simply a check list indicating what you want done and what format you would like your output in.  You can choose to either have your results printed out and placed in the envelope along with your test sheets, or to have PDF files of your results e-mailed to you.  The completed request form, correct answer key and student answer sheets are to be placed in an envelope (provided) and put in the Opscan Drop-Off Box located in MCT054.  Envelopes and blank request forms, along with a binder containing information on the various Output reports, are available at the Opscan Drop-Off Station in MCT 071.  Please have Opscan sheets turned and facing the same direction before submitting them.

Policy

  • It will be the sole responsibility of the user to submit a request as outlined in the submission standards listed above.

 

  • The user will deliver forms to the Opscan Drop-Off Box located in the the hallway to the right of the MCT 054 computer lab entrance, outside the CTC doorway and pick-up output of all scanning service from the Faculty/Staff Help Desk located in MCT 071.  Exams may be picked up during the following hours:  

                  Monday - Friday        7:00AM - 5:00PM

 

  • Opscan packets not picked up or returned via campus mail within 60 days of being processed will be shred.
  • Every effort will be made to preserve testing security, but submission procedures must be followed.

NCS Scanning Programs

Student Answer Sheets:  Unless otherwise instructed the ONLY part of the answer sheet that should have any marks filled in are the name, unique identifier (not required) and answer areas.  All other areas should be left blank.

Correct Answer Key:  Unless otherwise instructed the ONLY part of the correct answer key that should have any marks filled in are the faculty's name and correct answers. All other areas should be left blank.

 

Preferred Output

Email PDF:  Adobe PDF files of your requested Output Reports will be sent to you via e-mail.  You can then pick up the original Scan forms from the Faculty/Staff Helpdesk located in MCT 071.

Printed:  Paper printouts of your requested Output Reports will be placed into the envelope containing your original Scan forms.  You can then pick these up from the Faculty/Staff Helpdesk located in MCT 071.

 

Campus Mail Return

You may select to have your exam sheets returned to you through the campus mail rather than coming in to pick them up from the Technical Support Center.  For this option, check the box and provide your signature.  This option is only available with PDF results.  If you have any of your results printed out, then you must pick your exams up from the Faculty/Staff Helpdesk located in MCT 071, they will not be placed in the campus mail.  

  

Request Type (GPAS Score)

Option 1: 1 Correct Answer Only
Key: Year Field - Fill in the 1 in the first column.
Student Sheets:  Nothing special has to be filled in.

        Option A: Multiple Points Per Question
         Key: Year Field - Fill in the 1 in the first column.
         Special Codes K - fill in 1.
         Special Codes L - fill in point value (2-9).
         Student Sheets:  Nothing special has to be filled in.

        Option B: Previous # of Points to be Added
             Key:   Year Field - Fill in the 1 in the first column.  
             Special Codes N, O, P - 3 digit possible points to add.
             Sex Field - Fill in the F.
             Student Sheets:  Special Codes N, O, P - 3 digit previous points.

        Option C: Base Point Adjustment
         Key: Year Field - Fill in the 1 in the first column. 
         Special Codes K - fill in 9.
         Special Codes N, O, P - fill in 3 digit base points
         Student Sheets:  Nothing special has to be filled in

Option 2: 1 Correct Answer and Multiple Point Sets  (Requires Multiple Keys)
        Key 1:   Year Field - Fill in the 1 in the first column.  
        Special Codes K - fill in 1.
        Special Codes L - fill in point value.
        Answer Section - fill in the correct answers for all questions worth this 1st point value.
        Student Sheets:  Nothing special has to be filled in. 

        Key 2:   Year Field - Fill in the 1 in the first column.  
        Special Codes K - fill in 1.
        Special Codes L - fill in point value.
        Answer Section - fill in the correct answers for all questions worth this 2nd point value.
        Student Sheets:  Nothing special has to be filled in.

Option 3: Multiple Versions of the Same Test  (Requires Multiple Keys)
Key: Year Field - Fill in the 1 in the first column.
Student Sheets:  Nothing special has to be filled in.

Option 4: Survey/Questionnaire
Key: No Answer Key Required.
Student Sheets:  Nothing special has to be filled in.

 

Requested Output Reports

Score Only (Printed on Student Form Only)
The score will be printed only on the student form.  No reports will be generated.

Printed List of Scores (NCS01)
This program generates a list of students and their scores (total number of correct answers) in the order in which they were scanned and this program also generates an alphabetical listing.  It should be run only if scanning was done using GPAS Score, Option 1 without Options A, B, or C selected.  These other scoring options do not produce a true score.  To get a true score one of the NCS programs on the mainframe needs to be run. Data files can NOT be created if the SSN is found on any of the opscan forms submitted.

Item Analysis (NCS20)
This report can be run with files scanned using GPAS Score, Options 1 through 3. 
Generates a report that shows for each question the number of responses for each answer.  The responses are divided into quarters (lower, lower middle, upper middle, and upper).  Total percent and point bi-serial correlation is also shown for each question.

Grade Report (NCS30)
This report can be run with files scanned using GPAS Score, Options 1 through 3.
Generates three lists - name order, unique identifier order (if used), grade order.  Lists show total points, percent achievement, rank, letter grade, z-score and t-score.  The grade assumes a normal distribution.

 

Mini Item Analysis (NCS32)
This report can be run with files scanned using GPAS Score, Options 1 through 3.
Generates a report that shows for each question the correct answer, number of responses and percentage for each answer, and point bi-serial.

Individual Student Response (NCS33 and NCS33B)
These reports can be run with files scanned using GPAS Score, Options 1 through 3.
Generates a report that shows for each student their response to each question and the correct answer if their response was incorrect.
3 per page OR Condensed (NCS33) - Displays as many students as possible per sheet.  Number of students per page varies depending on number of questions. Some student responses may span multiple pages.
1 per page (NCS33B) - Displays one student per sheet.  For exams with a great number of questions, two sheets may be generated for each student in order to display all responses.

 

Survey/Questionnaire Results (NCS40)
This report can be run with files scanned using GPAS Score, Option 4 only.  It does not make use of an answer key.
Generates a report that shows for each question the number of responses and percentage for each answer.

 

Email Scanner Output DAT File
This option allows you to request to have the data file, that was generated from the NCS Pearson Scanning Program, e-mailed to you.  This is not a PDF file but the raw data file from which the various reports listed above are generated.  This file does not contain any of the output reports.

CTC Opscan Request Form

  • The person requesting service will fill out the top and left side of the form.

 

  • The information under REQUESTOR INFO lets the CTC operator know who to contact with any questions regarding the submitted forms, as well as where to send any requested e-mail results.

 

  • The information under PREFERRED OUTPUT will inform the CTC operator of what format to prepare your Output Reports in.

 

  • The information under REQUEST TYPE will indicate to the CTC operator what NCS Pearson Scanning Programs are to be run.

 

  • The information under REQUESTED OUTPUT REPORTS will tell the CTC operator what computer reports are to be generated.

 

  • A CTC operator will fill out the information on the right side of the form marked ICTC USE ONLY.
