  Standard form for changing your major or minor. Directions included.
•  Request for Graduation Check  (Arts and Sciences majors only)--email version Students should request a graduation check when they have at least 75 credits earned to make sure they know exactly what is needed to complete graduation requirements.  Download this form, fill in, and email to the College of Arts and Sciences.
•  Request for Graduation Check  (Arts and Sciences majors only)--hard copy version
  PDF, hard copy; print, fill in, and bring to Dauphin 225.

  You need to get all necessary signatures and submit this to the dean's office no later than the end of the first week of classes for the semester you plan to receive credit.
• Request for Exception [doc] Submit this form if you are requesting an exception to a policy, a course substitution, or some other special waiver or request.  Email to Dr. Kim Long, kmlong@ship.edu.
