Student Form

• Change of Major/Minor Form  Standard form for changing your major or minor. Directions included.
• Internship Application  You need to get all necessary signatures and submit this to the dean's office by December 1 for spring internships, and June 1 for any summer or fall internships. 
• Request for Exception [doc] Submit this form if you are requesting an exception to a policy, a course substitution, or some other special waiver or request.  Email to Dr. Kim Long, kmlong@ship.edu. 
•  Request for Increase in Maximum Credits for Registration.  Email to pahoop@ship.edu.  
• Request for Individualized Instruction/Independent Study  Get all signatures, as well as syllabus, before bringing in person to the College of Arts and Sciences, DHC 225.  Note: resources are very limited, so these are evaluated on need (course not offered for graduation, etc.).
