Available to  

Qualifying families and individuals with income below $51,000  

Items to Bring  

  •  Proof of Identification (i.e. driver’s license) 
  •  Social Security Card and Birth Date for yourself, spouse, & dependents 
  •  Wages & Earning Statements(W-2, W-2G, 1099-R from all employers) 
  •  Bank Routing and Account Numbers for Direct Deposit 
  •  Interest and dividend statements from bank 
  •  Total paid for day care AND day care provider’s tax ID # 
  •  Current Year Local Tax Return 
  •  Copy of 2012 Tax Return 
  •  *Filing a joint return? Both spouses must be present to sign the required form*  

Earned Income Tax Credit (EITC) Eligibility  

1 . Investment Income less than $3,000

2. Earned Income (Salaries, wages, tips, net earning from self-employment) and adjusted gross income in 2013 is less than or equal to incomes below:

**This building is located to the left of the Luhrs Performing Arts Center, and in front of Reisner Dining Hall.  Parking is available in a small lot directly attached to the side of the building.**
