Academic Integrity Procedure
Fall 2025 Academic Integrity Violation Process Summary
When course instructors suspect a violation of the academic integrity policy, they are encouraged to follow the process and options outlined below:
Course Instructor/Student Resolution
Option 1: Choose not to submit the Academic Integrity Course Level Resolution Form.
Option 2: Submit the Academic Integrity Course Level Resolution Form and report the consequence to the student without a meeting.
Option 3: Course instructor meets with student to discuss the violation and associated consequence (s).
Upon completion of the discussion, the meeting and outcome is documented by use of the Academic Integrity Course Level Resolution Form. The course instructor reminds the student that they are provided up to 72 hours to respond via e-mail their decision to accept or reject the discussed resolution. The course instructor is required to select one of six options as indicated below.
The student:
- Accepts responsibility and the consequence (s): case resolved.
- Does not accept responsibility but accepts the consequence (s): case resolved.
- Did not attend the prearranged meeting and the consequence (s) stand: case resolved.
- Was provided up to 72 hours to consult with support system to decide on accepting responsibility and the consequence but failed to respond via ship e-mail. The consequence (s) will be implemented: case resolved.
- Does not accept responsibility and requests the department chair/ associate dean of the college to mediate a resolution.
- Selected to use the 72-hour option before making a decision. At the end of the 72 hours, the course instructor will complete the Student Decision After 72-hours Academic Integrity Form.
OR
- Course instructor decided to complete and submit the form without meeting with the student.
Upon completion and submission of the Academic Integrity Course Level Resolution Form during the meeting, the course instructor will receive an electronic copy of the form and must forward a copy to the student. The form guides the instructor to show the form to the student with the understanding that its submission attests to the student’s agreement with the accuracy of the information on the form.
Course Department Resolution
Option 1: The course instructor consults with the department chair/associate dean to arrange for a meeting that includes the department chair/associate dean, faculty and student in order to facilitate resolution. Submit the Academic Integrity Department Level Resolution Form during the meeting if all parties agree with the information on the form.
Option 2: The course instructor consults with the department chair/associate dean and agrees that the case should be moved directly to the College/School Level by the Academic Integrity Board. (This decision may be based on repeated violations, incidents involving more than one student or may be requested by the course instructor, department chair/associate dean, and or student.)
College/School Resolution via the Academic Integrity Board (AIB)
- AIB convenes and reviews all documents related to the case in preparation for meeting with all parties involved in the case, within 10 business days. The AIB, however, does not meet during the Winter or Summer term, and therefore cases are heard during the following fall or spring semester up until the last day of classes.
- An AIB meeting is held with the department chair, the course instructor and the student (s).
- AIB chair and committee determine the final disposition of the meeting (or case?) and submits the College/School Level Academic Integrity Board Resolution Form.
- Once the AIB resolves the case, the AIB chair receives a copy of the completed form documenting the resolution and forwards a copy to the department chair/associate dean, course instructor, and the student.
All submitted forms are stored electronically in the case management software (Maxient). Access to these forms is limited to designees in the Provost’s and Deans’ offices. Information about any student’s previous Academic Integrity violations can be requested of these offices. Send an email to your college dean with the student's name, SUID, and the reason for the request of the student’s previous Academic Integrity violations.
