• Declaration of Major, Minor, & Other Credentials: Standard form for changing your major or minor. Directions included.
• Request for Increase of Maximum Credits: Request to exceed the maximum credits for the Spring/Fall (18+ credits), Winter (8+ credits), or Summer 12+ credits) semesters.
• Withdraw from College: Fillable form to indicate you are withdrawing from Shippensburg University or taking a leave of absence.
