Residence Hall Work Order Form

Facilities maintenance requests submitted through this form are sent via email to your RD. The RD or RAs will take the information and enter it into the Facilities work request system. Work will be coordinated in your room to make the repairs in a timely manner. You can include your phone number in the Student Contact Phone Number field; this will be added to the work request by your RD or RA. The Facilities Department may contact you directly if there is a question or issue.

Forms submitted after business hours or on the weekend will be reviewed and submitted to facilities on the next university business day. If you have an immediate issue after hours (i.e. loss of power, water leak, safety concern such as a lost key or door that will not close/lock) please contact the RA on Duty.
